Instructions for Submitting Rosters
If you would like to turn in your rosters electronically, please fill out the Provider Agreement for ELC of Duval's Secure Website form with your current email address and fax it to 904-394-1235 .
Once your Provider Agreement for ELC of Duval's Secure Website form is received:
- You will receive an e-mail, at the email address you listed on the form, with your username and password.
- Use the username and password when logging into the secure upload database where you can submit your rosters.
